The DCSD Physical Form can be found under the "QUICK LINKS" click on "SPORTS PHYSICAL FORM" or under "FORMS" at the top of this page. You can easily access the registration site by toggling the "SPORTS REGISTRATION" tab at the top of this page.

PLEASE NOTE: Registration on this site is mandatory for participation in practice and tryouts for the 2024-2025 sports seasons. This includes a current physical uploaded to the Mountain Vista registration.
